Garnishments: What information do I need to file a bank garnishment?

You will need to file an affidavit for garnishment that has a one page attachment when the garnishment is on a financial institution (Summons of Garnishment Upon a Financial Institution). On the affidavit you will need to provide the names of the plaintiff, defendant, the garnishee, the amount due on the judgment, the court it originated from and the case number. On an attachment to the summons you will need to provide party names, any other names used by the defendant, social security number or bank account number, if available.
